Project Completion Form

Please complete this form after your charger has been installed and is operational. In addition to this form, you will need to email* the following to ev@unitil.com:

1. Photos of the charger and its unit/station identifier.
2. Your electrician's final invoice marked "Paid in Full" showing:
   • the installation address
   • the installation Electrician's name
   • MA Electricians License Number
   • Electrician’s address and phone number
   • the date the work was completed
   • the work that was completed
   • the cost of labor and materials.
